TIER 1
Patient and control testing
Patient and control testing with a full concierge plan. The most complete option.

TIER 2
Single-patient testing
The same concierge plan with testing on the patient only, with no control.

results
▸ Three 30-minute appointments, including an intake visit and phone calls

TIER 3
Concierge care, testing already done
For patients who already have their personalized testing results and want ongoing support.
▸ One 60-minute appointment to review your personalized care plan

results
▸ Three 30-minute appointments, including an intake visit and phone calls
▸ One set of paperwork or a letter of support
$5,000 concierge care
Includes access to member pricing on add-ons.
TIER 4
Single visit care plan
One visit and one care plan, for patients whose testing is complete and who don't need ongoing concierge care.
▸ A personalized care plan based on your completed testing
▸ A single visit to go over the plan
$2,500 one time

All prices are in US dollars. Concierge plans in Tiers 1 and 2 require a one-year contract.
